Self-directed Support is the care and support you agree and arrange after completing a Supported Self Assessment Questionnaire.
There are 4 options to choose from:
- Accept a payment from the council, then arrange, manage and pay for the support you choose.
- Ask the council to arrange, manage and pay for the support you choose.
- Ask the council to choose, arrange, manage and pay for the support they think is best for you.
- A mix of options 1 to 3.
Personalisation is the way we work together with you, ensuring that you have choice and control to make and shape your care and support.
If you think you need a level of care or support to help you in your daily living then you should contact your Health and Social Care local office. A friend, neighbour, doctor, family member, nurse, or another person can make contact on your behalf.
