Self-directed Support - my life, my care, my support

Self-directed Support is the care and support you agree and arrange after completing a Supported Self Assessment Questionnaire.

There are 4 options to choose from:

  1. Accept a payment from the council, then arrange, manage and pay for the support you choose.
  2. Ask the council to arrange, manage and pay for the support you choose.
  3. Ask the council to choose, arrange, manage and pay for the support they think is best for you.
  4. A mix of options 1 to 3.

Personalisation is the way we work together with you, ensuring that you have choice and control to make and shape your care and support.
