The Ayrshire Central Hospital Social Services team provide support to:

Day hospitals.

Psycho-geriatric services.

Acute geriatric services.

Douglas Grant Rehabilitation Unit.
Home from Hospital
Our home from hospital organisers provide intensive home care services and support to patients who reside in Ayrshire, who are being discharged from North Ayrshire hospitals.
Welfare Rights
Our welfare rights| officer can provide you with advice and advocacy regarding benefit issues and can offer representation at tribunals.
Duty Service
Our team provides a duty service every afternoon. We give advice on:

Community assessments.
-
Home care services.
-
Respite and day care services.
-
Financial matters.
-
Housing.
and via referrals to the Occupational Therapy Department

Assessments for Aids and Adaptations.
Links to other services
We also provide the link between the hospital and social work area teams and other council services throughout North Ayrshire, South Ayrshire and East Ayrshire.
Our teams also liase with outside agencies, including:

Care Home and Residential Care home staff.

Family representatives.
-
Housing officers.
-
Dementia groups.
-
Communication groups for elderly care.
-
Community Mental Heath Services.
-
Other community health staff such as District Nurses and GPs.
Other services
Some of the other services provided by the hospital team include:

Attending weekly ward reviews and multi disciplinary case conferences.

Arranging discharge planning meetings, Community Care Assessments, Home Care Services.

Providing information on Housing, Benefits, Respite, Day Care, etc.

Supporting Service Users families and carers.
-
Undertaking Mental Health Officer duties, e.g. completing social background reports.
-
Counselling and support services.
-
Promoting awareness of Community Care issues and the role and remit of hospital Social Workers.
